This includes the Chronic Disease program, which offers weekly appointments to ensure patients&#8217; well-being, and the Falls program, designed to assist individuals at<br>elevated risk of recurrent falls to receive the necessary support. During my 8-week internship, I analyzed the data collected by MHI from the beginning of the year through the<br>conclusion of my internship. This analysis led me to develop a dashboard that simplifies the understanding of patient data and enhances how MHI can better assist them. <\/p>\n\n\n\n<p>Additionally, my time at MHI has broadened my perspective on the community, exposing me to programs such as First Door and enlightening me on discussions regarding mental<br>health in Crawfordsville.
